Why December 31, 2026 matters
Medicaid can pay approved medical bills from before the day you apply. This is called retroactive coverage. NC Medicaid is shortening it for most adults on January 1, 2027.

| Item | Apply by Dec 31, 2026 | Apply from Jan 1, 2027 | Status |
|---|---|---|---|
| Months of back coverage | 3 months | 1 month | final |
Apply for NC Medicaid by December 31, 2026 and it can cover approved costs from up to 3 months before you applied. From January 1, 2027, adults 19 to 64 without a disability get up to 1 month.
If you had a hospital stay, surgery or other large bills in recent months and think you might qualify, applying by December 31, 2026 gives NC Medicaid more room to cover them.

New work rules for people who apply in 2027
Starting January 1, 2027, most adults 19 to 64 who apply for Medicaid through expansion may have to show 80 hours a month of work, school or volunteering, or $580 a month in income, for each of the 3 months before they apply. Many people are exempt, including parents of a child under 14 and people who are pregnant or have a disability.
These application rules start January 1, 2027. Once you're enrolled, the work requirement and renewals every 6 months apply to you too, unless you're exempt. See whether the NC Medicaid work requirements apply to you.
What to gather before you apply
- Proof of identity and North Carolina residence.
- Social Security numbers for everyone applying.
- Proof of income, such as recent pay stubs or a tax return.
- Information about any current health coverage.
- Recent medical bills, if you want Medicaid to look at back coverage.
Don't hold your application while you gather everything. You can usually send missing papers after you apply, and your county DSS will tell you what it still needs.
How to apply
- Online through ePASS, North Carolina's benefits application.
- In person or by mail at your county Department of Social Services (DSS).
- Through HealthCare.gov. If your Marketplace application shows you may qualify for Medicaid, it is sent to NC Medicaid.
Medicaid takes applications all year. If you aren't sure you qualify, apply anyway and let NC Medicaid decide. See do I qualify for Medicaid.
Applying early does not guarantee that every old bill will be paid
The application date affects how far back Medicaid can review eligible care, but a bill still has to meet the program's requirements. Ask DSS to review the relevant months and give the provider your application or coverage information. A hospital invoice alone does not establish that you qualified for Medicaid in the month of treatment or that every charge is a covered service.
Make a separate list for each month of unpaid care
Organize the provider name, date of service, account number and amount billed. Keep copies of statements and any insurance explanation of benefits. Note whether income, household circumstances or other coverage differed during those months. This helps DSS review the correct period and helps the provider identify which claim needs attention.
Example: Someone applying in December has bills from earlier in the fall. Rather than assuming all fall bills are included, the person asks DSS which months can be evaluated and supplies the requested eligibility records. The result depends on the application date, eligibility for each month and covered services. This example illustrates the review process, not a payment promise.
Before and after you submit the application
- Submit accurate information promptly. Save the confirmation number or dated receipt. If something is missing, ask DSS what to provide and by when.
- Keep contact information current. A notice sent to an old address can leave you unaware that additional information is needed.
- Tell providers an application is pending. Ask how they handle pending Medicaid claims and keep records of the conversation. Do not assume collection activity stops automatically.
- Review the written decision. Check the approved coverage dates and whether retroactive months were addressed. Follow the notice's appeal instructions if you disagree.
- Coordinate other insurance. Do not cancel existing coverage based solely on an application submission. Confirm eligibility and effective dates first.
Who can answer which question?
Your county DSS handles application documents and eligibility decisions. The medical provider handles its bill and claim submission. An insurance agent can explain alternative coverage if you do not qualify, but cannot approve Medicaid or guarantee retroactive payment.
