Medicare and weight-loss drugs in 2027
Medicare drug plans generally don't cover drugs used only for weight loss. The Medicare GLP-1 Bridge is a temporary CMS program that fills part of that gap. It started July 1, 2026 and runs through all of 2027.
Medicare GLP-1 Bridge dates
- July 1, 2026The GLP-1 Bridge begins.
- October 15, 2026 – December 7, 2026Medicare Open Enrollment. If you don't have Part D, this is when to add it for 2027.
- January 1, 20272027 drug coverage starts. The Bridge continues.
- December 31, 2027The GLP-1 Bridge is scheduled to end.
Which drugs the GLP-1 Bridge covers
Drugs covered by the Medicare GLP-1 Bridge
| Drug | Covered? |
|---|---|
| Wegovy (injection or tablet) | Yes |
| Zepbound KwikPen | Yes |
| Zepbound single-dose pens and vials | No |
| Foundayo (tablet) | Yes |
| Ozempic | Not under the Bridge. Part D plans may cover it for type 2 diabetes. |
You pay $50 for a one-month supply, at any income level. Extra Help doesn't lower it, and it can't be spread out through the Medicare Prescription Payment Plan.
Who qualifies
Medicare.gov lists four requirements. You must meet all of them:
- You have Part D drug coverage, through a stand-alone drug plan or a Medicare Advantage plan that includes drug coverage. Some special plan types don't qualify.
- Your drug plan isn't already paying for a GLP-1 for you. If it is, you keep getting it through your plan.
- You don't have type 2 diabetes, moderate-to-severe sleep apnea or fatty liver disease. If you do, your drug plan may already cover a GLP-1 for that condition.
- You're at least 18 and meet a BMI rule: a BMI of 35 or higher; or 30 or higher with certain heart failure, hard-to-control high blood pressure or chronic kidney disease (stage 3a or above); or 27 or higher with prediabetes or a past heart attack, stroke or blocked arteries in your legs or arms.
Your doctor also has to certify that you're using the drug along with a diet and exercise plan.
How to get it
- Talk to your doctor about whether a GLP-1 is right for you and whether you qualify.
- Your doctor sends the prescription to your pharmacy. The pharmacy checks your eligibility with your Medicare number.
- Your doctor submits prior authorization to Medicare. You get a letter when it's approved.
- Pick it up for $50. Refills don't need a new approval while you stay on the same drug, even if your dose changes.
How it fits with your Part D costs
The $50 copay sits outside your Part D plan's costs. It doesn't count toward your deductible or the $2,400 out-of-pocket cap for 2027. You still need Part D to use the Bridge, so keep paying your drug plan premium.
If you take other prescriptions, compare drug plans for 2027 during Open Enrollment the usual way. See the 2027 Part D out-of-pocket max and which drugs get lower Medicare prices in 2027. For coverage of Ozempic and Wegovy for other conditions, see does Medicare cover Ozempic or Wegovy.
