NC Medicaid renewals move to every 6 months

Starting January 1, 2027, adults 19 to 64 covered through NC Medicaid expansion must renew their Medicaid every 6 months instead of once a year. This change does not apply to American Indians or Alaska Natives.

How a 6-month certification period works (NC Medicaid's example)

  1. January 1, 20276-month renewals and the work requirement begin for expansion adults.
  2. January through JuneMary's certification period. If she isn't exempt, she reports work, school or volunteer activity for at least 3 of these months.
  3. End of JuneMary renews. Meeting the requirement keeps her Medicaid for the next 6-month period.

Your own dates depend on when your certification period starts. Your county Department of Social Services (DSS) will send renewal notices to the address it has on file, so keep that address current.

What you may need at each renewal

Every 6 months, NC Medicaid checks whether you still qualify. If you aren't exempt from the work requirement, you'll also need to show 80 hours of work, school or volunteering, or $580 in income, in 3 of the 6 months.

  • Pay stubs or income records for each month.
  • School records if you attend at least half-time.
  • Volunteer logs with dates and hours.
  • Exemption papers, such as an SSI or SSDI award letter, if you're exempt.

How to avoid a gap in coverage

  • Keep your address and phone number current with your county DSS so renewal letters reach you.
  • Mark your renewal month on a calendar. With renewals every 6 months, it comes around twice a year.
  • Keep records every month, not just at renewal time. It is easier to show 3 of the 6 months when the papers are already saved.
  • Answer DSS letters quickly. Renewal letters have deadlines, and a late answer can end your coverage.
  • Have a backup plan. Know your Marketplace options before your renewal date in case your Medicaid ends.

A few minutes each month can keep you from losing coverage over paperwork.

If you miss a renewal or lose Medicaid

Contact your county DSS right away if you get a letter you don't understand or missed a deadline. They can tell you what's needed to keep or restore your coverage.

If your Medicaid ends, you may be able to enroll in a Marketplace plan within 90 days. If it ended because you didn't meet the work requirement, you can't get help paying for that plan. See what happens if you lose NC Medicaid in 2027.
