Medicare drug plans cover these drugs for diabetes and certain heart conditions. Coverage for weight loss alone works differently. See the GLP-1 section below.
Will you pay the negotiated price?
Not directly. The negotiated price is what Medicare drug plans pay for the drug. What you pay at the pharmacy is still set by your plan: its deductible, its copay or coinsurance for that drug's tier, and whether the drug stays on its formulary.
If your plan charges coinsurance, a percentage of the price, a lower drug price can lower what you pay. If it charges a flat copay, your cost may not change much. Either way, everything you pay for covered drugs counts toward the $2,400 yearly cap. See the 2027 Part D out-of-pocket max.
What to check during Open Enrollment
- Is your drug on each plan's 2027 formulary? Plans can add or drop drugs each year.
- What tier is it on? The tier sets your copay or coinsurance.
- Does the plan need prior authorization or step therapy? Those rules can change for 2027.
- What is your total cost for the year? Add the premium, the deductible and your drug costs, not just the price of one drug.
Open Enrollment runs October 15, 2026 through December 7, 2026, and changes take effect January 1, 2027.
GLP-1 drugs for weight loss are a separate program
Part D plans generally don't cover drugs used only for weight loss. The Medicare GLP-1 Bridge, which runs July 1, 2026 through December 31, 2027, lets eligible people with Part D get certain GLP-1 drugs for weight loss for a $50 copay with prior authorization. That copay does not count toward the Part D cap.

Quick questions
Is Ozempic cheaper with Medicare in 2027?
The price Medicare plans pay drops to $274 for a 30-day supply. What you pay depends on your plan's deductible and cost sharing for that drug.
Do I need to switch plans to get the negotiated prices?
No. The prices apply in every Medicare drug plan that covers these drugs. Comparing plans still matters because each plan sets its own copays, tiers and formulary.
Do the negotiated prices apply to Medicare Advantage drug plans?
Yes. They apply to Part D coverage, including Medicare Advantage plans that include drug coverage.
