How your benefits work
You don’t have to meet a plan deductible or pay a plan copay before a covered fixed benefit is payable. Your network discount lowers the eligible provider charge, and your scheduled benefit helps pay the remaining amount.
Choose a participating doctor or hospital to access pre-negotiated rates. You can use other providers, but network discounts help your benefits go further. We’ll help you check your providers before you apply.
For a covered service, the plan pays the amount listed in your benefit schedule to the provider or to you. The payment is based on your selected benefit level, not a percentage of the bill.
Your network savings and scheduled payment work together to help cover the cost of care. If the benefit exceeds the eligible charge, the difference can be paid to you. Any charges above the benefit remain your responsibility.
Access a nationwide network with 58% average savings on combined inpatient and outpatient services, based on the network’s 2023 pricing data. Negotiated rates help your fixed benefits go further. Your savings will vary by provider, service, and location.
For a covered office visit, a participating provider’s negotiated rate can lower the charge, and your scheduled benefit helps pay that amount. If the eligible benefit is more than the charge, the difference can be paid to you. A large bill can exceed the benefit and you owe the difference. We’ll walk through the payment schedule and examples with your personalized quote.
Show your plan ID card. Participating providers file the claim and receive the applicable benefit payment to apply to your bill. When you use a non-network provider, you can submit the claim yourself. We can help you understand the process.
Benefits for everyday care and more
Available options include scheduled benefits for the services below. We’ll review the exact dollar amounts, visit limits, and state-specific terms with your personalized quote.
Benefits for covered primary care, specialist, and urgent care visits. Different plan levels offer different payment amounts and annual visit limits.
Scheduled benefits for physical exams, certain health screenings, and eligible immunizations. Age requirements and annual limits apply.
Benefits for covered hospital stays, emergency room care, eligible surgeries, and outpatient facilities. Payment amounts depend on the service and plan level.
Benefits for covered laboratory tests, diagnostic imaging, and eligible generic or brand-name prescription fills, subject to the plan’s limits.
Included access to unlimited general, non-emergency phone or video visits at no additional charge, 24/7. It’s a convenient way to discuss everyday health concerns wherever your schedule takes you. Specialty virtual visits can cost extra.
An included prescription discount program can help you compare pharmacy prices, alongside the policy’s scheduled prescription benefits. The virtual-visit service and discount program are separate offerings. Neither is insurance.

Make a confident decision
Bring your questions. We’ll talk through your doctors, medications, budget, and the care that matters to you, then explain how the plan’s benefits can work in your situation.
We’ll review your health history, prescriptions, and planned care with you. This program is medically underwritten and has a preexisting-condition limitation commonly running 12 months. Routine pregnancy and childbirth are generally excluded, with only complications potentially covered under policy terms. Fixed benefit insurance is not a substitute for comprehensive coverage. Eligibility, benefits, and terms vary by policy and state; we’ll explain the details before you apply.
You can start with a personalized base-plan quote, then bring your questions to us. A consultation gives you a chance to put the premium and benefits together, check your doctors, and explore optional accident, critical illness, and dental and vision benefits around your needs.
