What being in Catawba County means

One town, two hospital systems — and a plan list all its own

Two things shape what fits you in Hickory. The first is the county: Medicare Advantage and Part D availability is set county by county, and Hickory is Catawba County, a different list from anything in the Charlotte metro an hour to the southeast. The second is unusual: Hickory has two hospital systems in the same town — Catawba Valley Medical Center and Frye Regional Medical Center — plus care reaching toward Lenoir and Morganton. Because a plan built around one system will not automatically cover the other, the deciding question here is less about the town and more about which doctors, in which system, you intend to keep. We check that one at a time.

There is no single “best” path through Medicare. What fits depends on your doctors, your prescriptions, how much you travel, and what you want your costs to look like. Here is how we work through each piece.

Turning 65

Your Initial Enrollment Period generally runs seven months — the three months before your birthday month, your birthday month, and the three after. Two things catch people out. Enrollment is not always automatic: if you’re receiving Social Security benefits at least four months before you turn 65, Medicare generally enrolls you in Part A and Part B for you, and otherwise you usually have to act. And waiting until the birthday month itself can push your start date later than you expected.

Come with the doctors you intend to keep, your prescriptions with doses, and whether you or your spouse are still working. If Medicare itself is still new to you, start with how Medicare works; for timing, see when you can enroll in Medicare.

Retiring, or still working past 65

Whether you can delay Part B without a late-enrollment penalty depends on your specific situation, including whether you have group health coverage based on your own or your spouse’s current employment. Employer size can also affect how Medicare coordinates with that coverage, so we check the details before you decide.

Medicare Part B timing

Three questions come up constantly: when should Part B start, can I delay it while I’m working, and what happens the month employer coverage ends. We push people to ask early because the Part B late enrollment penalty, once it attaches, generally stays with you for as long as you have Part B. It is not a one-time fee.

Part D prescription drug coverage

Drug coverage is its own decision with its own timing. Going without creditable prescription drug coverage after you are first eligible can bring a late-enrollment penalty of its own, which is why “I don’t take anything right now” is not quite the whole answer.

Medicare Supplement (Medigap)

A supplement works alongside Original Medicare and helps with costs Original Medicare leaves behind. In North Carolina, Medigap plans are standardized by letter, so the basic benefits for a given plan letter are the same from one insurer to the next. Premiums, underwriting, discounts and company service can still differ — which is where the comparison actually happens.

This is worth a careful look in Hickory precisely because of the two-system situation. A supplement works alongside Original Medicare rather than through a network, so it is less sensitive to whether a given doctor is Catawba Valley or Frye. Medicare Advantage
