Enrollment is not always automatic: if you’re receiving Social Security benefits at least four months before you turn 65, Medicare generally enrolls you in Part A and Part B for you, and otherwise you usually have to act. And waiting until the birthday month itself can push your start date later than you expected.

Whether you can delay Part B without a late-enrollment penalty depends on your specific situation, including whether you have group health coverage based on your own or your spouse’s current employment. Employer size can also affect how Medicare coordinates with that coverage, so we check the details before you decide rather than working from a rule of thumb.

Three questions come up constantly: when should Part B start, can I delay it while I’m working, and what happens the month employer coverage ends. The reason we push people to ask early is that the Part B late enrollment penalty, once it attaches, generally stays with you for as long as you have Part B. It is not a one-time fee.

Drug coverage is its own decision with its own timing. Going without creditable prescription drug coverage after you are first eligible can bring a late-enrollment penalty of its own, which is why “I don’t take anything right now” is not quite the whole answer.
