Your Initial Enrollment Period generally runs seven months — the three months before your birthday month, that month, and the three after. Two things catch people out. It isn’t always automatic: if you’re drawing Social Security at least four months before 65, Medicare generally enrolls you in Part A and Part B; otherwise you usually have to act. And waiting for the birthday month itself can push your start date later than you expect.

Retiring, or still working past 65

Whether you can put off Part B without a late-enrollment penalty depends on your circumstances, including whether you have group coverage through your own or your spouse’s current job. Employer size can also change how Medicare coordinates with it, so we go through the specifics before you decide.

Medicare Part B timing

Three questions come up again and again: when should Part B begin, can I hold off while I’m working, and what happens the month employer coverage ends. Ask early, because once the Part B late-enrollment penalty attaches it generally stays with you for as long as you keep Part B. It is not a one-time charge.

Part D prescription drug coverage

Drug coverage is a decision of its own, with its own timing. Going without creditable prescription drug coverage after you first become eligible can bring its own late-enrollment penalty — which is why “I don’t take anything right now” isn’t quite the full answer.
